At Sota Chiropractic, we see practice members with a wide range of health backgrounds. On every visits, we want you to feel comfortable and confident in our ability to support you and your family in achieving your health goals. We encourage you to take a look at the steps you will take as a new practice member below.
One-on-One Consultation
Your first visit begins with a brief tour of the office so that you can begin to feel comfortable with the space. A team member will meet with you to review your health history, go over any current health issues and identify your health goals.
Our team will complete a complimentary health insurance benefits check for your convenience. We ask that you please bring a photo ID and your health insurance information with you at this visit so we can scan it in to our files.
To save time on your first visit, we welcome you to complete the appropriate form(s) below.
Neurological Assessment
To better understand your current state of health and guide our care plan recommendations, we will complete a neurological evaluation using Insight technology. We also utilize digital X-ray to gather structural images of your spine, as needed.
This information will allow our doctors to develop personalized recommendations based off of 2 things; our objective findings and your health goals.
Our doctors will thoroughly review your health history and chiropractic evaluation findings after this visit and prepare a personalized plan for care moving forward.
Personalized Recommendations
Your second and most important visit will be your Report of Findings. At this visit, Dr. Marcus will review your x-ray images, neurological evaluation and individualized recommendations for care.
We will review your health insurance benefits, if applicable, as well as affordable payment options. You always have a choice and voice in our office and will never feel pressured to do anything that makes you uncomfortable.
You will receive your first adjustment at this visit. This is the starting point of your journey to living the life you want!
